COVID-19 MANDATORY INFORMATION

 

 

 

Screening Questions

You will be asked questions including the following prior to making an appointment and again upon arriving for your appointment:

  1. Have you or anyone in your household been diagnosed with or had any of the following COVID-19 symptoms in the past 21 days?

    • shortness of breath

    • persistent pain or pressure in the chest

    • new confusion or inability to arouse

    • bluish lips, face, fingers, or toes

    • fever (100.3⁰F / 37.9⁰C or higher)

    • cough

    • chills

    • vomiting or diarrhea

    • body aches

    • sore throat

    • new loss of taste or smell

  2. Have you or anyone in your household been out of the country in the past 21 days?

Appointment Protocol

  1. Begin your appointment request by texting me to arrange a consult via Zoom (not necessary for touch-ups)

    • Text or WhatsApp me by SMS at 503-706-8641.

    • We will decide on the best service for you, and I will guide you through the necessary paperwork, booking process, and paying your nonrefundable deposit

    • Once your appointment has been accepted, you will receive further information.

  2. I can't guarantee how long your appointment will take.

    • I advise clients to take Lyft or Uber; you will not be able to leave the studio to move your car.

    • There are parking lots and structures nearby. Please research these ahead of time.

  3. No one is allowed on the premises except for you, the client.

  4. Please call or text me when arrive at 503-706-8641

    • You must wait in your car or outside the building until your appointment time.

    • Please be patient. Preparing for and disinfecting after clients now takes twice as long as before.

    • Once I'm ready, I will contact you and meet you at the front door or 510 NW Glisan St., 97209

  5. You may not enter unless you are wearing a mask.

    • If you do not have a mask, please let me know, and I will bring you one before you enter.

  6. Use the restroom before entering my studio.​

  7. Upon entering my studio:

    • I will take your temperature

    • You will need to show me ID for first-time tattoo clients.

    • Then you will need to put all of your belongings into a plastic bag, provided for you.

    • Be sure to turn off your phone. You will not be able to have your phone with you during the service.

  8. Once you have stored your belongings, you will be directed to wash your hands for 20 seconds.

    • If there are any forms for you to sign, you will need to use hand sanitizer before we proceed.

  9. You will need to wear your mask throughout the entire service.

    • If you are unable to wear a mask for medical reasons, I regretfully must decline to serve you.

These are serious symptoms. Please contact your health provider ASAP to arrange for testing.
